SCFPA Teen Recognition

      Sponsored by the South Carolina Foster Parents Association

The Teen Recognition Committee of the SCFPA will identify a teen that meets the criteria listed below and is deserving of this recognition for the contribution they make to their community, school or church.

The winner will be recognized at the SCFPA Annual Training Conference with a monetary award.


Criteria

  • high school teen in 9th through 12th grade
  • nominated by school personnel or other adult such as pastor, employer, foster parent or social worker
  • grade point average of 2.0 or better
  • involved in after school, community service, church or other worthwhile activities

To Nominate a Teen

  • Print out and complete the nomination form
    (Adobe Reader required.)
  • Briefly answer the following questions on a separate sheet of paper:
    1. Why is the person you are nominating worthy of this recognition?
    2. What is your relationship to the person being nominated?
    3. What specific "character traits" would you say this person demonstrates?
    4. Include any information that will help collaborate why this person is deserving.
  • Return nomination form and all information to:
